If you still require your claim to be reviewed you are required to make an application to WorkAon in writing.
Your application must state the decision you are disputing, the reasons why you are disputing the decision, the relevant section of the legislation under which the decision was made, your claim number and contact details.
A form is available from the WorkAon office, free phone 0800 185 400. The application for review must be lodged and received within three months of the date of this decision. A late application for review may be accepted under the following circumstances:
You were unable to apply within 90 days because of the severity of your injuries; Your agent unreasonably failed to meet the required deadline;
WorkAon did not explain the requirements of the application process and your obligations.
Non-Work related accidents
You must contact your manager and Health and Safety Service to let her/him know that you will be incapacitated due to a non-work related accident. You must also forward your medical certificate to the Payroll service immediately or fax (07) 834- 3663 following issue by your treatment provider.
Payroll will forward your earnings details to ACC so if you are going to be incapacitated for more than one week ACC will know how much to pay you. During the first week you are entitled to use any outstanding sick leave entitlement to compensate you for your loss of earnings. ACC will pay 80% of your weekly earnings after the first week of incapacitation.
Payments for Work and Non-work Accidents
If you are fully unfit for work, and are receiving 80% of your normal earnings, you have the option to make up your earnings to 100% of ordinary time from your sick leave balance, if you have any sick leave owing to you. To apply for 100% of ordinary time wages, contact Payroll Help Desk Phone 834-3647 or Ext. 7647, and ask for an “Authorisation to use Sick Leave” form. This form must be signed and returned to Payroll promptly, to enable Payroll to access your sick leave. If your claim has not been accepted as a work or non-work injury, you will be entitled to use your sick leave balance during your absence from work.
Check list, have you:
let your manager know about your accident immediately?
filled in an incident report form if your accident was work related?
if necessary, visit your treatment provider and complete an accident claim form (ACC 45) and advised your treatment provider to send the claim form to WorkAon?
If you are unfit for work, have you contacted the Health and Safety Service?
sent your medical certificates to Payroll without delay?
completed your time sheet, indicating the penal time you were rostered to work for the first week of incapacity if it was a work-related accident?
