Section F - ACC procedure
For work related accidents, inform your supervisor and complete an
incident form immediately.
Inform you treatment provider (i.e. Doctor, Physio, etc) that WorkAon is the Waikato District Health Board’s insurer.
On the day you are given the ACC45 “Acc Injury Claim form” fax or drop into Payroll Services. Fax no (07) 834 3663
If you have been given time off work please notify Health & Safety Service immediately by phoning ext 8608.
If you would like to use your sick leave to make up your payment to 100% please complete the “Accident Leave Policy Authorisation to use sick leave” form and send to Payroll Services.
For non-work related accidents, send the Employers copy of the ACC45 “Acc Injury Claim form ” to Payroll Services immediately.
“ACC work related or non work related must be indicated on the Employee Work Schedule.
Ensure the correct ACC code is used in Onestaff.
Any further medicals (ACC18) must be forwarded to Payroll Services
immediately.
