Test ordering (lab and radiology) and result acknowledgement
 This issue is comprehensively covered in registrar orientation documents: Electronic Result Acknowledgement The bottom line for all doctors is we all must bear responsibility for acknowledging all test (laboratory and radiology) results as soon as they are resulted. You are responsible for the tests you order on your patients, particularly your admissions, and must acknowledge them all. Every time you look at a result you must acknowledge what you can as you take action on the result of the test.
 Tests being ordered by inpatient teams on patients under their care with issues pertaining to your specialty must have the name of the duty/on call consultant and the name of that team on the request so the result can be directed to the appropriate person/team.
 You may not under any circumstances use patient stickers with “Emerg” on them outside ED. When ordering tests (Lab and Radiology) in ED relating to your patients, partciularly your admissions, you must cross out Emerg and put the name of your consultant and the team, for example “Paul Reeve, General Medicine” or “Rowan French, General Surgery”
 The lab will not under any circumstances accept histological (tissue) specimens with “Emerg” stickers on them unless the above actions have been carried out. If you send a histological specimen to the lab with an Emerg sticker on it and no named specialist you will be held responsible for any issues such as delay in processing or non-processing of specimens.
