Laboratory request forms:
The general request form can be used for all applications. Discipline specific request forms reflect common test patterns or profiles for those areas (e.g. Women’s, ED).
Laboratory request forms are ordered by wards/clinics directly from the print shop.
The shaded fields are MANDATORY. These are:
Full patient demographics (at least 2 identifiers i.e. full name
and NHI and/or DoB)
REPORT TO: Destination for responsible clinician
acknowledging results
COPY TO: Clinician wanting copy of report
COLLECTION DETAILS: Time/date of collection, Collector of
specimen
Legible AUTHORISER/REQUESTOR name/contact details
Consultant/Team name for inpatients
Clinical details
Anatomical site where appropriate
Sample type for non-blood samples
Failure to supply these may require the requestor or collector to come to the laboratory within two hours of specimen collection and sign a declaration form before processing of the specimen/s can occur. Specimens other than blood should also have the sample type and anatomical site of origin (when appropriate) noted. PLEASE MAKE ENTRIES LEGIBLE so that the Laboratory can get it right.

Test(s) may be added on to a previously procured specimen provided the time limit has not been exceeded for the viability of the test requested, the appropriate specimen is in the laboratory and there is enough specimen to do so.
Add-ons are requested using the yellow Add-on sticker on a new form which is sent to the laboratory via the lamson. Only wards that do not have a lamson can ring for Add-on requests.
Ensure you have completed the Add-on test required, Requestor name, Team/Consultant and the current laboratory number, if you know it.
If unsure of the correct specimen type or volume of sample required, please refer to the Intranet or contact the Laboratory before collecting sample to avoid possible recollects.
Note some samples also require special transport conditions e.g. not via Lamson tube, on ice.
The citrated coagulation tube (blue top) must be filled to the fill line. If insufficient blood is collected the sample cannot be tested as the liquid anticoagulant dilutes the sample causing inaccurate
